Pre-appointment assessment: 3+ hrs of clinician review of all patient submitted: Intake forms, Labs, Medication history, Supplement history, Physician notes, Dietary intake/food journals, Lifestyle/Activity/Sleep Questionnaires
90 minute 1:1 video/phone consultation to review your history and discuss client concerns and next steps.
Post-appointment health action plan design: Taking the client’s concerns and additional information into account, the consultant will put together a personalized protocol, tailored to the unique needs of the client including recommendations for nutrition, supplementation, lifestyle, and any additional testing recommendations (if needed). Plans are typically delivered within 3-5 business days
Simple client follow-up questions after plan delivery can be submitted via email. If the follow-up question involves new health information, medications, or test result analysis, please book a follow-up consultation.

***Once payment is submitted, an email will be sent to the client with instructions for next steps. Please complete and return all new client forms/questionnaires no later than 48 hours prior to scheduled appointment time. This email will also contain a calendar link to schedule their consultation.
